payable to DEPARTMENT OF STATE

FILING FEE of $25 (Cash ONLY) per Passport

What is needed?

Two (2) 2x2 photos with light color (studio) background
Certified copy of BIRTH CERTIFICATE
Old passport or naturalization certificate
CHILDREN under 16 MUST have a BIRTH CERTIFICATE. (Even with an old passport)
NOTE! Children under 16 MUST have BOTH parents present. Children 16 - 18 MUST have at least one (1) parent present.
